Checkout 2020-2021

 

SCAESIC #618 CHECKOUT TO DO LIST 2021

Complete all the items that apply to you at least three days prior to your checkout date. (Some items have earlier deadlines.)

The Final Checkout Form will be emailed to you 2 days prior to your Checkout Date if you have completed all of the necessary items.  See Checkout Enote for important dates specific to your base school/district.

 

  • IEPS

    • Send in all IEP paperwork to the SCAESIC #618 Interlocal 10 days prior to your checkout date

    • All annuals due before 09/15/2021 must be completed prior to checkout

    • Contact your IEP Coach with questions

 

  • ANTICIPATED SERVICES CHARTS

    • Ensure that all lines for next school year are on the Anticipated Services Chart with the new provider and/or building

    • If the next provider is unknown, please make sure the lines have been duplicated with your name and current building

    • If a chart needs to be fixed that is already locked, contact Angela Cook:  acook@sped618.org

 

  • PROGRESS REPORTS

    • Ensure all IEP progress reports are completed and sent home.  Providers will attest to their completion on the Final Checkout Form

 

  • STUDENT FILES

    • If retaining the student on your caseload, be ready to enter where the student files will be located over the summer on the Final Checkout Form

    • If students are moving to a different provider, please make arrangements to give the files to the new provider.  You will document that you did so on the Final Checkout Form

    • If you are not returning, student files should either be retained or given to the new provider.  You will document that you did so on the Final Checkout Form  

    • School Psychs will retain the graduated/moved files that would normally go to SCAESIC #618 Interlocal Coop.  (The new cooperatives will be able to tell you what you should do with those files next school year.)

    • SLPs will retain Speech Only dismissed/moved files that would normally go to SCAESIC #618 Interlocal Coop.  (The new cooperatives will be able to tell you what you should do with those files next school year.)

  • GRADUATING SENIORS

    • Complete ALL senior tasks listed in the email that you will receive from your IEP Coach.  Do not forget to complete and send in the ESC-267 Dismiss forms to the SCAESIC #618 Interlocal Coop

 

  • IMC MATERIALS

    • Respond to Jeanine Lavington's emailed request concerning IMC materials.  The IMC will discontinue checking out materials on May 1st.

 

  • PARA ITEMS

    • Ensure ALL inservice documentation is submitted - VERY important

    • Send in resignation if not accepting position for 21/22

    • Contact your new cooperative with para staffing requests/needs

 

  • INVENTORY

    • Complete and return the Inventory form you received in an email from Angela Cook:  acook@sped618.org

 

  • MENTORING LOGS

    • All mentoring logs must be submitted via the Google Forms or the Paper Forms to Cynthia Himes. You can scan and email the paper forms to chimes@sped618.org, or send them through the SCAESIC #618 Interlocal Coop van run. 

    • For questions about mentoring logs, please email Cynthia at chimes@sped618.org.

 

 

  • MEDICAID

    • Ensure that all Medicaid logs are completed.  Contact Kristie Pivarnik with questions:  kpivarnik@sped618.org

    • Each provider responsible for Medicaid logs will attest to their completion on the Final Checkout Form 

 

 

 

  • LEAVE IN SKYWARD

    • All leave (Professional, Emergency, Activity & Sick Leave) is entered in the SCAESIC #618 Interlocal Skyward

 

  • TECHNOLOGY

    • If you were assigned a SCAESIC #618 Interlocal device for your use and were contacted by Denise Ketcham, make arrangements with Denise to return the devices by your final checkout date:  dketcham@sped618.org

    • If you have SCAESIC #618 Interlocal owned technology devices that are used in your classroom, or are on a student’s IEP, those will stay with the student’s classroom, and/or the student.
